Why did we need this? (what does this change enable us to do)

This change is made purely to add a new issue template for GitHub issues.

What did it change?

Adds a new issue template

Should it be merged upstream - if not, when can we delete it?

No, this change is to be kept local only

What is needed to get this merged / deleted?

This should not be merged nor merged upstream and can be kept local

  • is the implementation work to make suitable for merging / deletion completed?
  • Is there an associated test?
  • is this currently part of the Conda package?
  • is this properly cleaned up in our local repositories?
